Hospital Discharge Specialists Team meeting to arrange efficient and comfortable patient discharge from hospital / rehabilitation facility to home


Supplementary services include:


  • Provision of all necessary equipment / assistive devices approved by the discharge team in order to ensure safe and painless process

  • Assistance with transportation (including wheelchair-accessible transport) on the discharge date

  • Post-discharge follow-ups and supervision

  • Coordination of medical services, including nursery care, personal support and other requests that require accomodation
